For complete historical records or official physical copies, please download the "Medical Records Release Form" from our Forms section, complete it, and submit it to our administrative office in person or via secure fax.
We require a minimum of 24 hours notice for appointment cancellations or rescheduling. This allows us to offer the time slot to another patient in need of care. Late cancellations or missed appointments without prior notification may be subject to a nominal fee.
